Complete an Emergency Environmental Control Plan for ALF

On March 26, 2018, Governor Rick Scott signed two bills into law (Rule 58A-5.036), and (Rule 59A-4.1265) requiring Assisted Living Facilities (ALF) and Nursing Homes to have an alternate power source such as a generator(s) capable of maintaining an indoor temperature of 81 degrees Fahrenheit or lower, for a minimum of 96 hours.

The following form collects information used by the City of Miami Division of Emergency Management for the review of your Emergency Environmental Control Plan. Review this form and carefully follow all instructions. The review process has become more stringent due to lessons learned from previous events.

All assisted living facilities (ALF) in The City of Miami must complete this form. 

You need to submit or renew this form every year. 

  • If you've previously submitted your plan and have no changes, you'll need to "renew" your registration each year. Follow the link below and select "no changes" on the first page. 
  • If you're making updates/changes to an existing plan, you'll need to submit a new registration.
