COVID Complaints

Did you see a business breaking the rules for COVID-19 reopening and would like to report them? Complete this form and The City will look into it. 

Examples of what this form should be used to report:

• Servers and cooks not wearing masks.
• Alcohol establishments without food service that are open that should not be open.
• Establishments not limiting their occupant loads to 50% of their approved maximum occupant load.

Examples of what the form should not be used for.

• General emergency order questions.
• Questions regarding future phases and business openings.
• Questions regarding testing, screening, etc. These can be referred to the City of Miami COVID-19 Call Center - (305)-960-5027.
• Reporting of fire violations, unsafe structures, code compliance issues, etc.
