User Testing Feedback Form

Hi, would you like to participate in a short study today to improve the City's customer service?  This is our new TEST website.  It wont take more than 20 minutes

TIPS: If they ask you how to get through a page, say "what do you think you should do?". And if they seem frustrated/shy, say, "Nothing you say is going to hurt my feelings". 

Also, don't probe with questions about specifics, but you can ask, "Why did you make that decision?"

You can say things like "hmmm", "I see what you did that" "If I weren't here, what would you do next?" "You're doing great!"
