This link leads to the machine readable files that are made available in response to the federal Transparency in Coverage Rule and includes negotiated service rates and out-of-network allowed amounts between health plans and healthcare providers. The machine-readable files are formatted to allow researchers, regulators, and application developers to more easily access and analyze data.

 

MEDICAL coverage is through CIGNA POS Open Access. The coverage also includes alternative medicine, wellness, home healthcare and more.

DENTAL Cigna 

DHMO:

  • In-network coverage only
  • Preventive care services – most are covered at low cost or no cost to you.
  • Fees you pay your dentist are clearly listed on your Patient Charge Schedule (PCS).
  • No deductibles
  • No dollar maximums

DPPO:

We have two types of vision coverage. One is free/embedded in the medical plan.

We also offer the opportunity to enroll in a more comprehensive vision plan. 

A FSA is a spending account provided by your employer that can be used to pay for different types of eligible expenses.

Health Care FSA

Covers eligible health care expenses for you and your covered dependents.

Dependent Care FSA

Covers eligible expenses for dependents age 12 and under, and disabled dependents of any age.

  • Maximum contribution $2,650.00
  • Minimum contribution $250.00
  • Rollover maximum allowed is $500.00 of unused funds

Dependent Care FSA

  • Maximum contribution is $5,000.00
  • Minimum contribution is $250.00
  • Rollover is NOT Allowed

 

 

The City's CIGNA Behavioral Health benefits covers anxiety, depression, eating disorders, smoking cessation and more. Coverage includes office visits, outpatient, inpatient, substance use office visit, substance use outpatient, and substance use inpatient. 

Employees pay 0% in-network, 40% out-of-network. 

 

The City of Miami has consistently maintained a below-market employee contribution premium, without minimizing the richness of its coverage and plan offerings.

Premiums are also pre-taxed - which saves the employee greatly on their contribution. 

For retirees - to receive a monthly premium discount you must have Medicare Part A & B.  Fax a copy of your signed Medicare card to 305-416-1760.

 

ON-SITE MEDICAL PROGRAMS: 

The City of Miami offers several onsite wellness events to promote healthy living. These events are free to employees and eligible dependents, and take place at the MRC and other city locations. These include:

  • On-site free employee medical clinic open Monday-Friday. 
  • Mammogram checkups
  • Health fairs (consultations with chiropractors, optometrists, physical therapists, farmers markets and more).  
  • Dental exams
  • Vision exams
  • Health clinic with an onsite nurse
  • EAP
  • Onsite Cigna Representative
  • Wellness challenges with rewards

 

Your home and work life impacts your health. Our Employee Assistance Program (EAP) provides access to work/life resources, and licensed clinicians. They can help you cope with a wide variety of concerns, from family and financial issues to substance use, emotional health, and stress. EAP services are also available to anyone in your household.

Cobra provides employees and their dependents with the right to continue coverage under their current health and dental plan if terminated or other changes occur which result in the loss of coverage.

  • Dependents eligible for 36 months 
  • EE coverage as of 2022 is $740 monthly 
  • EE+ spouse $1629.84 monthly/medical 
  • EE+ children $1370.65/monthly medical 
  • EE+ family $2111.41/medical monthly

 

  • Executives who have elected not to be part of the Miami Pension Program may enroll in The City's 401k, which is employer matched.
  • Deferred compensation plan is an additional option for retirement savings. A deferred compensation plan allows you to contribute to a retirement account pre-tax.
    • Enroll at any time through their website
    • Contributions are made through payroll deduction and can be changed at anytime.
    • Pre-tax benefit reduces taxes each pay period.
