This page contains some of the application forms required by City departments for various purposes. Please note that some permits require original carbon copy forms to be filled out at your NET office. All forms and applications are provided in PDF format for downloading / printing. Building Permit	Required to construct, enlarge, repair, remove or demolish any building structure.
Class I Special Permit	Required where specified uses call for mandatory technical determinations to establish special conditions and safeguards
Class II Notification Registration Form	All neighborhood and/or homeowner associations that wish to receive a notice of Class II Special Permit applications in their areas should register with NET on a yearly basis by completing the registration form and sending it to their local NET Office.
Class II Special Permit	Required when specified uses and / or occupancies involve substantial technical issues involving planning policy.
Fire Safety Application	Required prior to opening a business in the City of Miami.
Garage Sales	Required to hold a garage or yard sale in the City of Miami.
Miami Resident Vehicular Protection Program (MVP) Application	Required to park your vehicle free of charge in one of the Miami Parking Authority's municipal garages during hurricane season.
Noise Waiver Request	Noise from a construction site is prohibited between the hours of 6:00 p.m. and 8:00 a.m. the following day on weekdays, or at any time on Sundays or holidays. A noise waiver can be approved by the City Manager or a designee, as deemed necessary.
Business Tax Receipt (Occupational License)	Required for engaging in any business, profession or occupation in the City of Miami.
Special Events Application	Required to hold an event on City of Miami property.
Supplemental Tree Removal Application	Required to remove or replace a tree in the City of Miami.
Supplemental Waste Fee Application	Required prior to opening a business in the City of Miami.
