Copies of the following must be submitted at the time of application:

• F.B.A.T, C.J.B.A.T or the FDLE Police Examination

• Valid State of Florida Driver’s License
• High School Diploma or GED

• Heart Bill Affidavit
• Background Investigation Release Form
• One of the following: birth certificate; naturalization certificate; valid U.S. passport
• Military Discharge papers (Form DD 214), if claiming veteran’s preference pursuant to F.S.S. 295
• Letter from Veteran’s Affairs or the Department of Defense dated within the last year that states the percentage of disability, if you are claiming employment preference for disabled veteran
