Passport Application Procedure
Adult Applicant (16 and older)

• Original birth certificate if born in the U.S. or original naturalization certificate if born outside the U.S., if U.S. passport was never issued, or was issued as a minor
• Previous U.S. passport, if one was ever issued
• Photo I.D. (U.S. Driver’s License or U.S. Identification Card)
• Two passport photos (Photo service is available at this location for a fee of $12.00)
• Check or money order, ONLY, payable to the “U.S. Department of State”

  Regular Service of 10 weeks for $75.00, OR
  Expedited Service of 4 weeks for $135.00

• Processing fee of $25.00 in cash, check or money order payable to the “City of Miami”
• Applicant MUST be present at the processing of the application

• Original birth certificate (Long form indicating parents’ names)
• Previous U.S. passport, if one was ever issued
• Parents’ U.S. Driver’s Licenses or U.S. Identification Cards
• Two passport photos (Photo service is available at this location for a fee of $12.00)
• Child MUST be present at the processing of the application
• For children 13 and younger, both parents should be present. In the event one parent is unable to be present, the other parent MUST complete and notarize a “Statement of Consent Form” accompanied by a copy of their U.S. Driver’s License or U.S. Identification Card
• Check or money order, ONLY, payable to the “U.S. Department of State”

  Expedited Service of 4 weeks for $120.00

• Processing fee of $25.00 in cash, check or money order payable to the “City of Miami”
